[RBS] survey for women who have used a doula

hello ladies of rbs!
i am currently training to be a proffessional birth assistant/ doula, and as part of my training i am doing a survey of 100 women who have used a birth assistant/doula during labor.
if you could take a few minutes to participate, i'd really appreciate it. the survey consists of about 10 questions.
