[RBS] Important information for members of Maccabi Magen Zahav and Clalit Platinum

Following recent changes in National Health Insurance legislation, Kupot
Cholim are NO LONGER able to extend cover to medicines NOT included in
the Health Basket.



Migdal (the oldest insurance company in Israel) has recently launched a
new plan to fill this void and provide cover for medicines not included
in the Health Basket.



During the month of June, there is a special opportunity for people who
have been members of either Maccabi Magen Zahav or Clalit Platinum from
before 31/12/2007 to sign up to the Migdal-Pharm plan with:

·        NO underwriting conditions

·        NO waiting period

·        NO exclusions on pre-exisiting conditions (up to the age of
85)

This is a once in a lifetime opportunity not to be missed, especially if
you suffer from a pre-existing condition.



The Migdal-Pharm plan will provide cover of 1 million NIS for medicines
not included in the health basket.



Monthly premiums are as follows:

Individuals:

·        Aged 25-35:   9 NIS per month

·        Aged 36-54:   14 NIS per month

·        Aged 54+:   24 NIS per month

Family Rates:

·        Eldest family member aged 54 or less:   19.5 NIS per month
for the entire family

·        Eldest family member aged 54 and above:   49 NIS per month
for the entire family

As mentioned before, this is an opportunity NOT TO BE MISSED, especially
for people with pre-existing conditions.
