[RBS] israeli passports

I need some information regarding applying for passports at Misrad Hapnim.
In order to apply for childrens' passports do both parents have to be there? Do the children themselves have to be there or are teudat zehuts (both parents) and pictures enough? 
We have teudat maavar for all of them now but they are expired, do we bring them along?
Any info would be greatly appreciated as I hope IY"H to make this a one time visit to Misrad Hapnim.
