[RBS] driver's license renewal?

I already went for an eye exam and was issued a green and white form that
also needed to be filled out and signed by my doctor.  What do I do with
this form now that it is complete?

Just about a month and a half ago my husband renewed his license but the
cost and procedure was different.  When he had his eye exam they processed
all his information there and submitted it to Misrad Harishui.  Does
someone have experience with this new process?
