When you book an appointment with the US Embassy, they will send you an email back.

You must CONFIRM THE APPOINTMENT or it will get canceled.

There is a link in one of the emails that they send, and it is NOT very clear.

If you don't click the link to confirm, the appointment will be canceled.

(The good part about this is that you can frequently find appointments coming up within a week for when people had their appointments canceled.)
