Hi David,
You might want to take a look at a tool called "Gursor maker" which
lets you do just that.

There's the spec
(http://www.freedesktop.org/wiki/Specifications/cursor-spec) if you
want to roll you own themes, and then there's the xcursor man page if
you want to read.
