actually, what the ad says is that if you sign up by dec 31, you will have
PRIORITY for transplants.
if you sign up later, you will have to wait 3 years for priority.

BTW, there is another card, called Bilvavi, which has more rabbinical
recognition, and is also accepted for the same purposes
